Neoclassical economists say that price is set by the marginal utility of a good to the buyer. That’s true, as far as it goes. But taking that language according to its ordinary meaning, we get the extremely misleading, simplistic impression that price is set by how much something’s worth to us. “Marginal utility” carries a very specific technical meaning: The utility to the consumer of the last (or marginal) unit consumed. Marginal utility depends on the total supply and demand under the spot conditions of the market. When goods are reproducible and there are no artificial constraints on production, the supply will tend to fluctuate in response to demand until the amount demanded equals the amount supplied. The last unit supplied sells at a market clearing price equal to the cost of production.

We only see reproducible goods priced at their absolute utility to consumers when things go wrong — i.e., when output is artificially restricted or the seller is a monopolist, and thus able to price the good based on how much the consumer needs it rather than how much it cost to produce. This is called monopoly pricing or price discrimination. The classic example of setting a price based on what something is worth to the buyer is the $10,000 glass of water sold to a desert traveler dying of thirst.

Big Pharma defenders argue that patents are necessary because marginal cost pricing won’t repay the large initial R&D investment for developing drugs. But most drug R&D actually goes to gaming the patent system — developing “me too” versions of drugs whose patents are about to expire, or testing to secure patent lockdown on every major variant of a drug — not developing new drugs.

The other side of monopoly pricing is targeting the price to individual consumers’ ability to pay. In traditional manufacturing this was called “dumping.” A company would run its lines at capacity to minimize unit costs, selling as much of the output at the monopoly price. Then they would sell what was left over at whatever lower price less affluent consumers could afford — often dumping part of it at less than the cost of production. We see the analog of this in drug commercials all the time: “If you’re unable to afford [drug], [company] may be able to help.”

Medicare D and Obamacare amount to subsidizing people who can’t afford to buy glass of water insurance so that they’ll be able to afford to pay the water salesman $10,000. They amount, therefore, to massive subsidies to the drug companies and to healthcare providers. Thanks to government, Big Pharma is able to price gouge an even larger share of the population who would previously have been able to pay.

The proper approach is to remove the monopoly — in the case of drugs, by eliminating the patents on them. Unfortunately that is unlikely to ever happen regardless of which major party is in power. It would be an unthinkable assault on the basic foundations of corporate capitalism in a country where the state has always — even (or especially) during “progressive” times like the New Deal — been intimately connected with corporate power.

Instead the most viable solution is likely to be bypassing the unholy alliance between the state and the pharmaceutical industry, using the rapidly developing capabilities of do-it-yourself biotech to produce illegal open-source knockoffs of expensive drugs. In every area of economic life, capitalists depend on state-enforced monopolies and enclosures of one kind or another. But the rise of people’s technologies like cheap, small-scale means of production are making such monopolies increasingly unenforceable. The days of the Lords of Scarcity are numbered.

Komt tot zien: het Amerikaanse Hooggerechtshof heeft gestemd ten gunste van de individuele-obligatie clausule uit de door Barack Obama geïntroduceerde Affordable Healthcare Act, ook wel ObamaCare genoemd. Vanaf 2014 zullen alle Amerikanen dus een deel van hun belasting rechtstreeks aan een verzekeringsmaatschappij van van hun keuze moeten betalen in plaats van aan de staat (Of je de staat komt bij je langs).

En nu worden we overdonderd door de verwachte kritiek vanuit het rechtse kamp – je weet wel, dezelfde mensen die zorgden voor HMO’s, Medicare Deel-D en RomneyCare, allemaal in de naam van de vrije onderneming. Het dwingen tot het kopen van verzekering, of het nou voor jezelf of voor iemand anders is, is alleen omstreden wanneer de Democraten of een New-Yorkse maffia familie het uitvoert.

Eigenlijk zouden de Republikeinen de komst van CannibalCare… sorry; ObamaCare… moeten toejuichen. Niet alleen is de grondvesting van deze nieuwe regeling gelegd door drie decennia van Republikeinen, niet alleen is de nieuwe regeling gebaseerd op het voorstel van niemand minder dan de Republikeinse presidentskandidaat Mitt Romney, niet alleen was de doorslaggevende stem in het Hooggerechtshof afkomstig van de door George W. Bush aangestelde, Chief Justice John Roberts, het is ook nog eens de ultieme overwinning voor de privatisering.

Huh? Wat? Ja, “privatisering”.

De primaire functie van de Amerikaanse federale overheid – net als die van alle politieke overheden – is altijd geweest om zo veel mogelijk rijkdom uit uw zak te stelen en deze over te dragen aan hun politieke vriendjes en de bevoorrechte elite.

Normaal gesproken word deze diefstal en resulterende herdistributie bereikt door complexe goochel trucjes, met veel poespas vanuit de politiek die het stelen van uw zuurverdiende geld proberen goed te praten met speeches over “publieke goederen” en “legitieme overheidsfuncties” waarna het geld overgemaakt kan worden aan houders van defensie-contracten, privé gevangenissen, wegenbouw bedrijven met vriendjes binnen het Ministerie van Infrastructuur en Milieu, enzovoort.

De huidige veranderingen binnen de Amerikaanse zorg zijn het hoogtepunt van het privatiseringsproces. In plaats van het oude systeem van achterkamer-politiek en gecompliceerde wetten betalen Amerikanen nu dus direct een deel van hun belasting aan zorgverzekeraars als Kaiser Permanente of Blue Cross Blue Shield. En net als met het normale belasting systeem, krijg je er misschien nog iets voor terug… of je het nou wilt of niet.

De meeste mensen waarmee ik gesproken heb over CannibalCare… sorry; ObamaCare… realiseren zich dat er zich een fundamentele morele fout schuil houdt achten het hele concept. Maar tegelijkertijd zijn ze bang voor de altijd maar stijgende kosten in de zorg, zonder zich er van bewust te zijn dat deze stijgende kosten het resultaat zijn van meer dan een eeuw aan ‘succesvolle’ politieke bemoeienis, en niet de schuld van ook maar iets dat te vergelijken is met marktkrachten. Daarbij realiseren ze zich niet dat al deze bemoeienis net zo vaak verdedigd word door “links” als door “rechts”.

De voormalige Republikeinse president George W. Bush stelde Medicare Part-D voor en tekende deze uitgebreide regeling. Republikeinse president Ronald Reagan tekende de Emergency Medical Treatment and Active Labor Act. Republikeinse president Richard Nixon bedacht en tekende de Health Maintenance Organizations Act. Tik een willekeurige Republikeins congreslid op zijn schouder en hij zal heldhaftig roepen dat hij Medicare wilt ‘redden’ en Medicaid wilt ‘decentraliseren’ door middel van lumpsumfinancieringen van uw geld. Vraag hem of hij geïnteresseerd is in het terug roepen van wetten die een middeleeuwse gilde (The American Medical Association) een monopolie garanderen op het geven van zorg, en uw zult vriendelijk gevraagd worden door de beveiliging het gebouw te verlaten.

Dus als een Amerikaanse burger denkt dat het stemmen op een Republikein (of een andere kandidaat wat dat betreft) komende November er voor zorgt dat CannibalCare… sorry: ObamaCare… teruggeroepen word, dan moeten ze nog maar eventjes goed nadenken. Zelfs als de volgende groep populaire politici een uitgebreide media voorstelling verzorgen om de Amerikaanse burger anders te doen denken, zullen de Democraten en de Republikeinen allebei doorgaan met het vinden van manieren om nog meer geld uit de portemonnee van de burger over te hevelen naar de bankrekeningen van hun politieke vriendjes. Want dat is wat de overheid doet. Wilt u betaalbare zorg, dan is de eerste stap er één richting het compleet afschaffen van de overheid.
